Patient Rights and Responsibilities
Choose ONE of the following Patient Registration Form Options below. The first one allows you to just print and then you hand write your answers. Where as the second form type allows you to fill in the form on the computer and then print.
OPTION 1: Patient Registration Form (PRINT ONLY TYPE) print this form and bring back with you completed on your appointment date)
OPTION 2: Patient Registration Fill in Form (FILL IN and PRINT) print this form and bring back with you completed on your appointment date)

Consent for Controlled Substance Required for new patients currently on a controlled substance (Schedule II, III, IV and V) and current patients that have been asked to complete
Authorization to Release Medical Records Who needs to complete this form? See the reasons below:
1.) If you have seen a previous psychiatrist or other mental health provider, please complete for each provider.
2.) If you have recent lab order results from your primary care physician (PCP), please complete with your PCP contact information.
3.) If you have been asked by our office to complete this form.
