Insurance Information for 2023
This document shows a list of most insurance plans we either accept and are in-network OR some plans we do not accept. The list is separated by Medicare type plans and Commercial (non-medicare) plans. Also note that there are exceptions to a few of the insurance company plans, so please read the list carefully. Please make sure you read the 'Notices" section at the end of the list. We request that you call your insurance plan and ask them specifically about your outpatient mental health benefits and provisions. Please read this 'Insurance Info' document as it has important questions for you to ask your insurance carrier.

We are not accepting any new Florida Medicaid (traditional), out of state Medicaid plans or any Medicaid HMO plan.

Medicare Open Enrollment Period: October 15, 2023 - December 7, 2023

Every year a Medicare Open Enrollment Period, correctly referred to as the Annual Enrollment Period, is made available for people who qualify for Medicare benefits. During the Annual Enrollment Dates, anyone who is eligible for Medicare Parts A and B can enroll in or make changes to their Medicare coverage.

 

This includes being able to change from Original Medicare to Medicare Advantage (sometimes referred as Medicare Replacement Plans, "Part C" or "MA plans"), change back to Original Medicare from Medicare Advantage, switch from your current Medicare Advantage plan to a different Medicare Advantage plan, make changes to your Medicare Part D coverage, or apply for Medicare Part D even if you didn't when you originally applied, although some fees may be incurred. 

 

If you decide to enroll in a Medicare Advantage plan, please make sure you check out our insurance list to see if we accept the plan and if your provider is in-network. I would recommend calling us before you sign up on any new plan or if you have any questions.

 

ACA Open Enrollment Period: November 1, 2023 - January 15, 2024

Auto Insurance and Worker's Compensation Claims

 

Unfortunately, we do not provide services for these types of cases. 

 

Appointment Reminders

 

At the time of your check-out at the office, we offer and provide you an appointment card. We encourage you to save this in a place for you to remember; and possibly add the appointment to your calendar book, smartphone or other electronic device. We also have an automated appointment reminder system.  This is provided as a courtesy.  Remember, you are responsible for attending your scheduled appointment. If you do not get your reminder call, text or email from our automated system, this is not an excuse for missing your appointment. If you do not contact us to cancel your appointment within 24 hours of your scheduled appointment you will be charged the Missed Appointment fee of $60.00. 

 

Cancellation and No Show Policies

 

It is our goal to be on time out of respect for both your and our time. To help keep us running on time, please arrive at least 5 minutes earlier than your scheduled appointment. There may be times that your provider is running late and this is most likely due to special circumstances with a prior patient appointment. We apologize for these incidences, but our goal is providing quality care to our patients. On occasion, there will be understandable reasons for missing appointments, but exceptions to this policy will be rare. Cancellation policy is standard in the medical and mental health fields and will be enforced.

 

 

Cancellation

 

If you cancel 24 hours prior to your appointment, there is no penalty. This will allow us time for the slot to be filled by another patient. We do request that you call the office at (941) 488-8884 to reschedule. If you cancel less than 24 hours prior to your appointment, there is a cancellation fee of $60.00. Excessive cancelled appointments may result in termination from the practice. 

 

How do I cancel my appointment to avoid a fee?

 

You can cancel your appointment by:

 

  • Calling the office directly at (941) 488-8884 at least 24 hours prior to scheduled appointment.
  • Cancelling in person 24 hours prior to scheduled appointment by coming into the office. 
  • Cancelling via the Patient Portal.
  • Cancelling via response to the automated appointment reminder system. We ask that you please call the office to verify your cancellation processed and updated to our appointment system and to reschedule your appointment. 
No Show / Missed Appointment
 
If you have an appointment scheduled and do not appear for the appointment at all, this is called a 'No Show'. No shows will be charged a $60 fee. After three (3) No Shows, you may be sent a letter of termination from the practice due to excessive missed appointments.
 
If you are a new patient to our practice and NO SHOW for your first scheduled new patient appointment you will be charged a $60 fee for your NO SHOW new patient appointment. Your Initial Visit Deposit will be applied to this missed appointment fee. If you have cancelled or missed your new patient appointment three times, unfortunately we will not be able to schedule you with our office and you will need to find a new clinician for your mental health treatment. 

Prescription Refill Policy

 

We ask that you call your pharmacy for your prescription refill requests. Our office is set up to receive and send prescriptions electronically. So, please let your pharmacy know that we accept eRx refill requests. If for some reason your prescription refill request through the pharmacy has not been handled and processed within 8 hours of your initial request, then please contact our office directly to inquire about the order.

 

Please read the following guidelines on rx refill request:

  • Prescriptions are all sent electronically, and on rare cases may be written or faxed, at the time of your appointment with sufficient quantities and refills, if necessary, to last until your next appointment.  If by chance you run out of medication before your next appointment, please call your pharmacy to request a refill be sent electronically.
  • Refill requests received after 3:00 p.m. may not be sent to the pharmacy until the following business day.
  • Please allow up-to 24 hours before you refill order request is processed.
  • Prescription refill requests for Schedule II medications (i.e. Ritalin, Adderall, Concerta, Dexedrine, Mydaysis, Vyvanse, etc.) requires that you have an appointment, at minimum, every 3 months if you are prescribed a stimulant medication.
  • Patients that have been prescribed controlled medications are requested to use the same pharmacy each time for refills.  Patients may be required to sign a Controlled Substance Contract with this office.
  • Controlled Substances history will be reviewed using the state of Florida Prescription Drug Monitoring Program database.
  • Controlled Substance prescriptions will only be sent for 30 day supply. We DO NOT send 90 day / 3 month supply for controlled substance medications to local or mail order pharmacy.
  • Contact your pharmacy to verify your prescription is ready for pick up before calling our office.
